Horizon Health Center has been a private, non-profit
provider of affordable, accessible, community health care for thirty-five years. Horizon offers low income
women, men and children coordinated access to prenatal care; post partum care; family planning and
preconceptual counseling; nutritional care; adult medicine; pediatric services; geriatric health care;
specialized gynecological services; immunization services; substance abuse education,counseling and
referral; HIV/AIDS education, counseling and testing; mental health care; dental services; teen services; and
parenting program.
Horizon offers sliding scale fees and accepts most insurance and HMO plans, as well as Medicaid; no one
is turned away because of inability to pay. Half of Horizon's clients are on Medicaid; 45% have incomes
falling between Medicaid eligibility and the ability to afford private insurance.
Located in the heart of Jersey City. Horizon is open at
convenient hours Monday through Saturday, including evening hours. Accessibility and coordinated services
give clients a real chance to find help and continue
healthy behaviors.
Horizon Health Center has a strong relationship with
the community in which it provides services. Most staff and board members live in the immediate community.
Similar to the clients served, Horizon staff is comprised of African-Americans; Africans; Filipinos; Indians;
Russians and other Eastern Europeans; Pakistanis; Asians; and white of various ethnicities.
Like the patients, staff members speak English, Spanish, Italian, French,
Tagalog, Russian, Polish and Arabic. Horizon's clinical care takes into account the special language, financial, social service and medical needs of this population.
Horizon also operates a large Teen Community
Center, which is adjacent to our three clinical buildings. The Teen Center runs groups on pregnancy prevention
and parenting, and offers programs in job training and study skills.
Horizon's ability to provide a multitude of on-site services, as well as linkages with other community
agencies, ensures that an entire family can receive the support necessary to be healthy, and to bear
healthy children.
